ABSTRACT

OBJECTIVE@#To compare the therapeutic effect between cotton-moxibustion and compound flumetasone ointment, and observe the effect on quality of life in patients with chronic eczema.@*METHODS@#A total of 66 patients with chronic eczema were randomized into an observation group (33 cases, 2 cases dropped off) and a control group (33 cases, 2 cases dropped off). In the observation group, cotton-moxibustion was adopted on target skin lesion, once a day, 3 cones a time. In the control group, external application of compound flumetasone ointment was given twice a day. The treatment for 3 weeks was required in the both groups. Before treatment and 1,2,3 weeks into treatment, scores of visual analogue scale (VAS), eczema area and severity index (EASI) and dermatology life quality index (DLQI) were observed, and the recurrence rate was evaluated in the follow-up one month after treatment.@*RESULTS@#Compared before treatment, the VAS scores of 1,2,3 weeks into treatment, the EASI and DLQI scores of 2,3 weeks into treatment were decreased in the both groups (@*CONCLUSION@#Cotton-moxibustion can effectively improve the pruritus symptom, skin lesion and quality of life in the patients with chronic eczema, the therapeutic effect is superior to the external application of compound flumetasone ointment.

ABSTRACT

Based on the previous study of compound liquorice microemulsion, this paper aims to prepare the compound liquorice microemulsion gel and investigate its pharmacodynamics of chronic eczema. The type, dosage and adding method of gel matrix, and formula dosage of humectant were optimized by single factor method to obtain the formula and preparation technique of the gel. With glycyrrhizic acid, glycyrrhetin and oxymatrine used as evaluation indexes, the Franz diffusion cell method was adopted to monitor the in vitro release profile of the gel. Eczema model of delayed-type hypersensitivity in mice was chosen to detect the ear swelling rate, degree of inflammatory cell infiltration of ear pieces, and pathological changes of ear pieces, so as to investigate the therapeutic effect of the microemulsion gel. The preparation process of the compound liquorice microemulsion gel was stable. The release of glycyrrhizin and oxymatrine was most consistent with the Hixcon-Crowell kinetic model, while the release of glycyrrhizic acid was most consistent with the Ritger-Peppas kinetic model. The pharmacodynamics studies proved that compound liquorice microemulsion gel could significantly reduce the ear swelling rate in mice, with good anti-inflammatory effect as well as the ability to resist the pathological changes of chronic eczema and inhibit the infiltration of dermal inflammatory cells. Therefore, the preparation process of compound liquorice microemulsion gel is feasible, with stable drug release and a significant therapeutic effect on chronic eczema.

ABSTRACT

The meridian system is a popular concept in traditional Chinese medicine. Meridians are channels through which the life energy (qi) flows and are important in herbal medicine as pathways associated with drug entry and distribution. This report describes a patient with chronic eczema successfully treated with Japanese (Kampo) herbal medicine, keigairengyoto (containing the Angelica dahurica root). A woman in her 40 s presented with a 3-year history of eczema on both hands to upper arms that was refractory to treatment. Her eczema was observed to be distributed along the lung meridians of hand-taiyin (the channel for the A. dahurica root), and keigairengyoto therapy was initiated after which her eczema significantly resolved. Knowledge of meridians and the concept of channels of entry of herbal medicines may be useful in diagnostic and therapeutic management of several diseases.

ABSTRACT

Objective:To observe the anti-inflammatory effect and its mechanism of Euphorbiae Ebarcteolatae Radix bacteriostasis ointment in eczema mice induced by 2,4-dinitrochlorobenzene (DNCB). Method:A total of 40 ICR adult mice were randomly divided into control group,model group,hydrocortisone Butyrate cream group (0.09g·kg-1) and Euphorbiae Ebarcteolatae Radix bacteriostasis ointment group (0.09 g·kg-1), with 10 mice in each group. Except for the normal group, other groups were given DNCB to induce the chronic eczema model. Twenty-four hours after DNCB stimulation, they were given the corresponding drugs through auricle and back, twice a day for 10 days. After drug intervention, efforts were made to measure the change of thickness and weight of the middle ear, assess the allergic effect, and calculate the spleen index of the mice. Optical microscope was used to observe the pathological changes in ear tissues of mice. And the levels of serum interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α), interleukin-2 (IL-2), interleukin-4 (IL-4) in mice were determined by multiplex immunoassay. Result:Compared with control group, the thickness and weight of right ears, score of allergic effect, spleen index and the levels of IL-2 and IL-4 in serum showed significant increases in model group (P<0.05,P<0.01). The histopathology injuries of ear were aggravated. Compared with model control group, Euphorbiae Ebarcteolatae Radix bacteriostasis ointment could reduce ear thickness and score of allergic effect, regulate the spleen index, decrease the inflammation factor in serum such as IL-2 and IL-4 (P<0.05,P<0.01), and improve ear histopathology injuries. Conclusion:Euphorbiae Ebarcteolatae Radix bacteriostasis ointment may have a good effect on eczema.

ABSTRACT

Anhidrotic ectodermal dysplasia (AED) is a rare hereditary disorder with a triad of sparse hair, dental hypoplasia, and anhidrosis. Here we report a case of AED with food allergy and atopic eczema. The patient was a 11-month-old boy admitted to our hospital with pyrexia for 2 weeks. He presented with a history of dry skin, eczema, and food allergy to egg. On clinical examination, his body temperature was 38.8°C, with dry skin and eczema almost all over the body, sparse eyebrows, and scalp hair. Laboratory investigations and physical examination did not show any evidence of infection. Radioallergosorbent test was positive to egg yolk, egg white, ovomucoid, milk, house dust, and house dust mite. As the child did not sweat despite the high fever, we performed the sweat test which revealed a total lack of sweat glands. Genetic examination revealed a mutation of the EDA gene and he was diagnosed as AED. His pyrexia improved upon cooling with ice and fan. His mother had lost 8 teeth and her sweat test demonstrated low sweating, suggestive of her being a carrier of AED. Atopy and immune deficiencies have been shown to have a higher prevalence in patients with AED. Disruption of the skin barrier in patients with AED make them more prone to allergic diseases such as atopic eczema, bronchial asthma, allergic rhinitis and food allergy. Careful assessment of the familial history is essential to differentiate AED when examining patients with pyrexia of unknown origin and comorbid allergic diseases.

ABSTRACT

Objective:To investigate the dose-effect relationship of Xianfu ointment and its decomposed recipes the 1-chloro-2,4-dini-trochlorobenzene(DNCB) induced chronic eczema in mice, and confirm the median effective dose (ED50) of each formula and the synergetic effect by compatibility. Methods:DNCB was used to induce chronic eczema in C57 mice. The mice were treated with gradient dosages of the Xianfu ointment (11.71-11 662.50 mg?kg-1?d-1,k = 0.316), Anemone flaccid (0.53-530.12 mg?kg-1?d-1,k = 0.316), Xianfu ointment without Anemone flaccid (11.18-11 132.40 mg?kg-1?d-1,k =0.316),respectively. The pathological features were observed after hematoxylin-eosin staining. The volume ratio of epidermides and the number of lymphocyte infiltrated in dermis were analyzed with morphometry. The serum levels of IL-2,IFN-γ,IL-4,and IL-13 were detected by ELISA assay. The ED50was calculated by non-linear regression with various slope using Prism-5.0 software.Results:The effects of Xianfu ointment and its decomposed recipes on chronic eczema showed a dose-dependent tendency. The dose-response curves showed"S"shape. The efficacy of Xianfu ointment on chronic eczema was the most significant among the three formulas, which was demonstrated by decreased epidemical thicknes (ED50= 377.90 mg?kg-1?d-1), reduced infiltrated lymphocyte number(ED50= 153.20 mg?kg-1?d-1), increased serum IL-2(ED50=608.90 mg?kg-1?d-1) and IFN-γ (ED50= 205.50 mg?kg-1?d-1) levels, and decreased serum IL-4(ED50= 198.70 mg?kg-1?d-1) and IL-13 levels (ED50= 117.60 mg?kg-1?d-1). And the dose-effect curves of Anemone flaccid and Xianfu ointment without Anemone flaccid groups were both right shift when compared with that of Xianfu ointment. Conclusion:Xianfu ointment and its decomposed recipes can effectively treat chronic eczema. Anemone flaccid has obvious compatibility synergy in the whole formula. The effects of Xianfu ointment is most significant.

ABSTRACT

Objective: To observe the effect of oxymatrine ointment on chronic eczema in mice, and preliminarily explore the mechanism. Methods:Thirty-two Kunming mice were randomly divided into four groups including oxymatrine ointment group, blank model group, blank ointment group and positive drug group treated with compound dexamethasone acetate cream. Eczema skin was con-tinuously treated with drugs for 14 days. The mice were sacrificed on the 15th day, and the eczema skin was clipped from back to make histological sections. The changes of inflammation were observed, and the inflammatory cell count was obtained. Meanwhile, heart blood was collected, and serum was obtained by centrifugation. The serum levels of IL-4 and IL-1β were determined by ELISA. Re-sults:The inflammatory cell count in oxymatrine ointment group and the positive drug group was lower than that in the blank model group and the blank ointment group (P0. 05). Conclusion: Oxymatrine ointment has certain anti-inflammatory effect on eczema, and the mechanism needs to be studied further.

AIM:To observe clinical the effects of Huashenzilu on mice models with acute and chronic eczema.METHODS:Eczema was induced by repeated local exposure to DNCB,and then the mice received Huashenzilu.Using ELISA kit method to test the serum concentration of IL-2,IL-4,TNF-α,IFN-γ,and HE staining to test histopathological changes.The ear swelling and scratching index were measured,and ear allergy score were measured according to the standard for evaluation.RESULTS:After the treatment of Huashenzilu,the serum concentration of IL-4,TNF-α significantly increased and the serum concentration of IL-2,IFN-γ significantly decreased in the chronic mice compared to the model group (P < 0.01).The ear swelling,scratching index and ear allergy score were significantly decreased.The phenomenon of epidermal thickening,acanthosis layer cavitation,and mast cells,macrophages infiltration were improved compared to the model group.CONCLUSION:Huashenzilu can control the chronic eczema in mice through the mechanism of regulating the balance between Th1 and Th2 immune response.

A total of 36 rats were divided into four groups of 9 randomly including oral medicine group, external inunctum group, combination therapy group, and model control group respectively. Besides, the blank group of 4 healthy rats were set. The oral medicine group was given Traditional Chinese Medicine Jianpi Chushi decoction [(Poria cocos, Chinese yam, Cortex dictamni, Zaocys dhumnade, Rhizoma atractylodis, Pericarpium citri reticulatae, Scutellaria baicalensis, Radix Sophorae Flavescentis, Raw Radix Paeoniae Alba, Licorice roots (Northwest Origin)] by gastric infusion (1.6 g/mL·5 mL/d); the external inunctum group was given Qingpeng ointment on the skin, the combination therapy group was given Jianpi Chushi decoction by gastric infusion and Qingpeng ointment combination therapy. The model control group was given normal saline (NS) of the same volume by gastric infusion and vaseline on skin. Continuous administration 15 d and stopped for 3 d. The thickness difference and weight difference of left and right ear of every group were measured and the degree of ear swelling were evaluated. The CD4+ and CD8+ content and the IL-2, IL-4 level of serum were detected, and the inflammatory cells counts of back skin were recorded.@*RESULTS@#After treatment, the degree of ear swelling of oral medicine group, external inunctum group and combination therapy group significant decreased compared with model control group (P < 0.05). The CD4+, CD8+ cell content and IL-2 level of oral medicine group, external inunctum group, combination therapy group and model control group significant decreased compared with blank group, and IL-2 level and the inflammatory cells count increased. After 15 d of treatment, the CD4+, CD8+ cell content and IL-2 level of serum of oral medicine group, external inunctum group and combination therapy group raised and the IL-4 level and the inflammatory cells count had significant decreased compared with model control group, and the effect of combination therapy group was more obvious (P < 0.05).@*CONCLUSIONS@#Qral Jianpi Chushi decoction could treat chronic eczema effectively, and oral Chinese medicine combined with ointment could enhance and speed up the efficacy.

ABSTRACT

OBJECTIVE:To observe clinical efficacy and safety of thymosin in adjunctive treatment of chronic eczema,and to investigate its mechanism from the change of peripheral T lymphocyte subsets. METHODS:160 patients with chronic eczema were randomly divided into observation group and control group,with 80 cases in each group. Both group received conventional therapy as mizolastine,Desonide cream. Observation group additionally received Thymosin enteric-coated tablet 20 mg,po,tid;control group was given placebo the appearance of which was same with Thymosin enteric-coated tablet. Treatment course lasted for 4 weeks. The change of T lymphocyte subsets,clinical efficacy and the occurrence of ADR were compared between 2 groups. Re-currence rate of both groups were followed up for 1 year. RESULTS:The effective rate of observation group(91.25%)was signifi-cantly higher than that of control group(77.50%),with statistical significance(P0.05). There was no statistical significance in the incidence of ADR between 2 groups(P>0.05). Re-currence rate of observation group(31.51%)was significantly lower than that of control group(64.52%)during follow-up period, with statistical significance (P<0.05). CONCLUSIONS:In summary,thymosin can improve clinical efficacy and reduce recur-rence rate by regulating T lymphocyte subsets with good safety.

ABSTRACT

Objective To observe the effects of Jianpi Yangxue Qufeng Formula (JPYXQF) on the AQP3 in mice with chronic eczema, and explore mechanism of action. Methods Fifty healthy male mice were randomly divided into 5 groups, namely normal group, model group, positive medicine group and JPYXQF high and low dose groups. Low-dose DNCB and Sennae Fominm were used to establish mice models of chronic eczema with spleen deficiency. JPYXQF groups were treated by JPYXQF for gavage, while the positive medicine group was treated by levocetirizine hydrochloride for gavage. The expression of AQP3 in mice skin tissue was detected by immunohistochemical method. At the same time, the pathological changes of skin were observed. Results The pathology of mice skin lesion showed that JPYXQF has certain recovery effects on the inflammation injury of skin lesion. Compared with the normal group, expression of AQP3 over expressed in model group. Compared with the model group, the expression of AQP3 in all treatment groups significantly decreased, and the staining intensity decreased. In the model group, the average optical density of AQP3 was significantly higher than that in the normal group (P<0.01). Compared with the model group, the treatment groups can reduce the expression of AQP3 in mice skin tissues (P<0.05). Conclusion JPYXQF can reduce the over expression of AQP3 in skin lesion, which is probably its mechanism for the treatment of chronic eczema.

ABSTRACT

Objective The article was to observe the clinical efficacy of the application of terbinafine and mizolastine in com -bined treatment of chronic eczema ( CE) with dermatophytes infection , so as to define the etiology role of dermatophytes in allergic dis-eases. Methods All subjects were randomly divided into experiment group and control group .The experiment group was treated with the combination of terbinafine and mizolastine , while the control group took mizolastine orally alone .EASI grading , recovery rate and effective rate were evaluated at 2, 3 week after the treatment and EASI grading and recurrence rate were evaluated at 4 weeks after the treatment. Results 79 patients had finished the experiment .Significant difference was found in the effective rates between two groups at 3 weeks after treatment (P<0.05).At 4 weeks after the treatment, EASI value and recurrence rate in experiment group were obviously lower than those in control group (P<0.05). Conclusion Good therapeutic effect has been achieved through the ap-plication of terbinafine and mizolastine in combined treatment of CE with dermatophytes infection , which implies dermatophytes plays an important role in the etiology of CE .

ABSTRACT

Leprosy is a chronic granulomatous disease affecting the skin and peripheral nervous system. Because of its variable manifestations, leprosy can be misdiagnosed as syphilis, sarcoidosis, psoriasis and eczema. A 73 year-old man showed erythematous papules on his arms and legs. He had been erroneously treated as eczema before the correct clinicopathological diagnosis of borderline lepromatous leprosy was made. We report this case to suggest that dermatologists should pay careful attention while diagnosing new cases of leprosy.

ABSTRACT

Objective To observe the clinical efficacy of herbs by internal and external administration combined with semiconductor laser irradiation in the treatment of chronic eczema.Methods Eighty-six patients with chronic eczema were divided into the herb group and the laser group by the randomized digital table,43 patients in each group.The patients in the herb group received oral administration with Chinese herbs together with application of herbs on the affected foci.The patients in the laser group received the same treatment as in the herb group and meanwhile treated with additional semiconductor laser therapy.The clinical effects were evaluated according to their itch degree and skin lesions.Result The cure rate and total effective rate were both significantly higher in the laser group than those in the herb group(P<0.05).Conclusion Compared with herbal internal and external administration only,the clinical efficacy of herbal internal and external administration combined with semiconductor laser irradiation is more effective in the treatment of chronic eczema.

ABSTRACT

Botryomycosis is a chronic, granulomatous and suppurative infection. The skin is the most frequently affected organ. The most common species is Staphylococcus aureus, followed by Pseudomonas aeruginosa. Clinically, the skin lesions most commonly affect the hands and feet and they can appear as nodules, ulcers, sinuses, fistulae and verrucous plaques. Histopathologic examination reveals grains with surrounding eosinophilic material and inflammatory cells. We report here on a case of botryomycosis at the excoriation site in a 90-year-old female patient with chronic eczema.

Objective To investigate the therapy of chronic eczema patient by mounting method withshen que point,and its effect to relieve itch fast and increase the comfort and contentment of patients.Meth-otis 68 caiges of chronic eczema were randomly enrolled into the experimental gttmp(38 cases)and the controlgroup(30 cases).The experimental group added mounting method with shen ClUe point to normal therapy in-observed the beginning time of itch remission and the therapeutic effect of two groups after 14 days.ResultsCompared with the control group,We found that itch remission in the experimental group wok effect earlier thanthat of the control group,P<0.05.and the therapeutic effect in the experimental group wag significantly differ-ent from that ofthe control group after 14 days.P<0.05.Conclusions The mounting method with sben quepoint can relieve pruritus of patients with chrunie eczema effectively and promote therapeutic effect.

ABSTRACT

Objective:To observe the therapeutic effect of narrow-band UVB combined with glycyrrhizin compound on chronic eczema. Methods:81 cases of patients with chronic eczema were randomly divided into treatment group 42 cases and the control group 39 cases . The treatment group was treated with narrow-band UVB therapy once every two days combined with compound glycyrrhizin capsules 150 mg,3 times a day;the control group was treated only with compound glycyrrhizin capsules 150 mg,3 times a day. Two groups were treated for 4 weeks altogether. Results:The efficacy rate of therapy group (95%)was obviously better than the control (64%).
